About Our Imaging Service

Easy Steps to Submit a Case:

1. Click on "Submit a Case" button and fill out the form.

2. Attach your photographs as Shown above (we do not image retracted views)

3. Add your treatment plan with specific teeth numbers to be treated, the desired shade, whether you will alter the soft tissue and on which teeth. You can provide any other specific information you’d like us to have, or you can say “provide the ideal smile design” and leave the details of design to us.

4. Click on "Make a purchase" to pay for your case

5. We will acknowledge the receipt of the case and provide the date you can expect to have the digital photos and document. We will send you the imaged photos as well as a document (see below) with your practice information, the patient name and date. Above each side-by-side photo there will be a disclaimer that reads "This is a computer simulation of the proposed treatment".
